Benefit Program Descriptions

Medical Coverage

Employees may enroll in a comprehensive medical plan administered through United Healthcare - Choice Plus PPO a Preferred Provider Organization. Benefits include, but are not limited to, the following: preventive care, inpatient/outpatient services, therapy services, home health care, and more. The coverage provides both in-network and out-of-network providers. Your out-of-pocket expense is lower if you use an in-network provider; however, you may still get a reduced benefit with an out-of-network provider. The cost of the plan is shared by Franklin County Cooperative Health Benefits Program and the employee, and the premium contributions are made on a pre-tax basis. Prescription Drug Coverage

Employees enrolled in the medical plan will automatically receive prescription drug coverage through OptumRx. You will receive a prescription drug card to be used at participating local pharmacies. Franklin County Cooperative Health Benefits Program offers mail order through OptumRx. Both retail and mail order options are available. You save both time and money by obtaining your ongoing routine prescriptions through the Mail Order Program. You can also save on your copay expense by using generic medications.

Dental Coverage

Franklin County Cooperative Health Benefits Program offers dental coverage administered through Aetna. Employees have a choice between two dental plan options - Dental PPO or DMO. Both plans include preventative care covered at 100% and coinsurance benefits for basic and major restorative services. The dental DMO, a Dental Maintenance Organization - only provides in-network coverage. The dental PPO - Preferred Provider Organization provides both in-network and out-of-network coverage. The use of in-network providers reduces your out-of-pocket expenses.

Vision Coverage

The vision benefit provides coverage at both in-network and out-of-network providers. The vision program is administered through Vision Service Plan (VSP). The plan includes coverage for an annual eye exam and a benefit toward contacts or lenses - for eyeglass lenses every 12 months and frames every 24 months. The benefit program also offers extra discounts on contact lenses, eyeglasses, sunglasses and laser vision correction. Click on the annual vision plan summary for more program details.

Employee Assistance Program

The EAP offers confidential support for everyday challenges and is available 24 hours a day 7 days a week. Services are available to any member of your household, but you must be in a benefit eligible employment classification. You are not required to be enrolled in the benefits package to receive EAP services. Your EAP benefit allows eight free counseling sessions per presenting problem per year through United Behavioral Health. This benefit is provided at no cost to you.

Behavioral Health Coverage

For services that go beyond those provided by the EAP, if you are enrolled in the benefit package, your behavioral health benefit is provided to you through United Behavioral Health. The coverage is available for both in-network and out-of-network providers. You will use your medical ID card to access the behavioral health benefits. Flexible Spending Accounts

Flexible Spending Accounts (FSA) allows eligible employees to use pre-tax dollars to pay for out-of-pocket medical, prescription drugs, dental, vision and/or dependent or elder care expenses. This is a 100% voluntary program and you do not have to be enrolled in the health plan in order to participate. The FSA plan year - or coverage period - begins January 1, and ends December 31. Disability Insurance Program

The Franklin County Cooperative offers a group disability insurance program to all benefit eligible employees. If you are eligible to enroll in the Cooperative's healthcare plan, you are eligible to participate in the disability insurance program.

The disability insurance program offers the following coverage:

  • Short Term Disability (STD) replaces a portion of your pre-disability income during the initial weeks of a disability. It pays a weekly benefit based upon your pre-disability income and provides benefits up to 26 weeks (approximately 6 months) after an initial waiting period of 14 days.
  • Long Term Disability (LTD) replaces a portion of your pre-disability income during an extended illness or injury. After an initial elimination period of 180 days (or until your Short Term Disability Insurance benefits end) it pays a monthly benefit based upon your pre-disability income. Benefits are paid up to your normal retirement age or Reducing Benefit Duration.

Life Insurance Coverage

Term life insurance is provided at no cost to full time benefits eligible employees the first of the month following your new hire waiting period. Employees have the option of purchasing additional insurance coverage. The following types of life insurance are available:

Basic Life: basic group term life that pays a $50,000 benefit in the event of an employee's death due to illness or injury. This is provided at no cost to the employee.

Accidental Death & Dismemberment (AD&D): AD&D benefit is also available and pays a $50,000 benefit in the event of an employee's death resulting from an accident. The amount payable is a percentage of the $50,000 AD&D benefit, determined by the loss. This is provided at no cost to the employee.

Supplemental Life: supplemental life is additional coverage an employee may purchase on the lives of their spouse, eligible dependent children and themselves. Coverage for the employee and spouse is through payroll deductions and purchased in $10,000 increments. The employee's eligible dependent children may have $5,000 or $10,000 in benefit coverage.
