Meet Therese Lentz and Carmen Long.
They are your Health Engagement Nurses through UnitedHealthcare and can help you navigate topics such as:
- Health promotion and prevention through sound health care decisions and education.
- Navigation of UnitedHealth Care tools and resources and referral to appropriate wellness programs and services.
- Health education to individuals and groups.
- Support for chronic conditions such as diabetes, heart disease, hypertension, asthma, and many others.
- Finding health care providers that fit your needs.

Nurse Therese has over 35 years of experience with a focus on cardiovascular disease, prevention, and wellness. She is married with 2 sons and 1 daughter, all of whom are married, and she has 5 grandchildren. In her free time she enjoys going to live music & sporting events, the beach, and spending time with family & friends.
Here's a little more about Carmen:
Nurse Carmen feels privileged to have been a part of the health and wellness journey of countless people as they navigated challenges and life changes over the span of her career. She has worked in cancer, orthopedic, cardiac, and critical care environments. She is married with two adult daughters. In her spare time, she enjoys spending time with family, sewing, walking outdoors, biking, and camping. She is very excited to work with the Franklin County Cooperative.
