Gym Membership Reimbursement
We are offering gym membership reimbursement to encourage you to take steps to improve your health. Physical activity is an important part of overall health and well-being: we want to help you get and stay fit!

Who is eligible to submit for reimbursement?
All benefits-eligible employees and benefits-enrolled spouses/domestic partners in the Franklin County Cooperative are eligible to receive reimbursement. One reimbursement per household EXCEPT households with 2 eligible FCC employees (i.e- spouses or parent/dependent child).
How do I apply for a gym reimbursement?
Complete and submit the Gym Membership Reimbursement Online Submission Form. You will need to provide proof of payment for your membership and proof of attendance at your gym. Please ensure your name is listed on both your proof of payment and proof of attendance.You will only be reimbursed for those months you can verify payment and activity.
How much will be reimbursed? How often do I have to go to the gym?
For each month in which you visit a gym(s) on at least 8 different days, you will be reimbursed the cost of your membership, up to $50 per month. It must be the same person visiting the gym each time. We will not reimburse annual fees. One reimbursement per eligible membership.
You can combine visits from two gyms to meet the 8 visit per month requirement. Please note, you must show proof of payment and proof of attendance for EACH gym. The maximum reimbursement amount remains $50 per month.
What if I miss the submission deadline?
We will not extend any deadlines. We will only reimburse for activity in the date ranges listed for that quarter. If you do not submit a form by the listed deadline of any quarter, you will miss out on the reimbursement opportunity. The online submission form will close at 11:59 p.m. on the submission deadline date.

What types of gym expenses are eligible for reimbursement?
- Full-service gyms that offer a variety of physical activities and exercise equipment including LA Fitness, Planet Fitness, Snap Fitness, Crunch Fitness, local recreation centers, etc.
- Boutique-style gyms or studios including Jazzercise, Curves, Orange Theory, Yoga, Kickboxing, etc.
- Personal trainers
- At-home/virtual memberships (i.e. Peloton)
Note: all memberships, including at-home memberships and personal trainers, are subject to the same requirements listed above (proof of visits, proof of payment).
How/when will reimbursement be paid to me?
Gym reimbursement payments will be made on a quarterly basis. Your reimbursement will be processed through payroll and included as additional taxable income on your paycheck. Please keep in mind the following criteria for issuing incentive payouts:
- ThriveOn cannot issue incentive payouts to anyone who is not in an active payroll status.
- Incentive payouts are made on a pre-determined schedule throughout the year. ThriveOn cannot issue incentive payouts outside of this schedule.
