Eligibility Criteria
You are eligible to apply for reimbursement if you meet the following criteria:
• Benefits-eligible employees and benefits-enrolled spouses/domestic partners in the
Franklin County Cooperative.
• One reimbursement per household, except for households with two eligible Franklin
County Cooperative employees.

Reimbursement Details
Reimbursement Amount:
• You can receive
up to $50 per month for gym memberships.
Attendance Requirements:
• 0-3 visits per month: No reimbursement.
• 4-7 visits per month: 50% reimbursement (up to $25).
• 8+ visits per month: 100% reimbursement (up to $50).
One reimbursement per household per month, except for households with two eligible employees.
Combining gym visits:
If you visit two gyms, you can combine visits. You will need to provide proof of payment and attendance for each gym. The maximum reimbursement amount is $50 per month.

Submission Deadlines
It is important to submit your reimbursement request by the deadlines. The online submission form will close at 11:59 p.m. on the due date.
What if I miss the submission deadline?
We will not extend any deadlines. We will only reimburse for activity in the date ranges listed for that quarter. If you do not submit a form by the listed deadline of any quarter, you will miss out on the reimbursement opportunity.

Reimbursement Payment Process
Gym reimbursement payments will be made on a quarterly basis. All incentives are paid through the employee paycheck and are taxed as normal income. Please keep in mind the following criteria for issuing incentive payouts:
1. ThriveOn cannot issue incentive payouts to anyone who is not in an active payroll status.
2. Incentive payouts are made on a pre-determined schedule throughout the year. ThriveOn cannot issue incentive payouts
outside of this schedule.

Eligible Expenses for Reimbursement
• Full-service gyms (e.g., LA Fitness, Planet Fitness, YMCA)
• Boutique-style gyms (e.g., Orange Theory, Yoga, Kickboxing)
• Personal trainers
• At-home/virtual memberships (e.g., Peloton)
Note: All memberships require proof of payment and proof of attendance.
