Vaccines & Boosters
Science has shown that vaccines and boosters are the best method of protecting yourself, your family, and your community against COVID-19 infection, serious illness, and death. Everyone 5 years and older is eligible to get a free COVID-19 vaccination and a free booster shot (including the new omicron booster).

Testing
To make testing as easy as possible, there are multiple opportunities for you to obtain COVID-19 at-home test kits.
If you are enrolled in the Franklin County Cooperative health plan, you can obtain up to 8 at-home tests per month through your pharmacy benefits. Coverage depends on where the test kits are purchased. Test kits must be intended for use by members covered under the health plan and not for employment-related COVID-19 testing requirements.

A preferred pharmacy*. Currently, the preferred pharmacies are:
-Walmart Pharmacy
-Sam's Club Pharmacy
-Rite Aid Pharmacy
-Walgreens Pharmacy

What to present? |
Your OptumRx id card at the pharmacy counter. Ask to have your test kits submitted to your pharmacy plan. A written prescrption from your doctor is not needed. |

What to submit to OptumRx? |
Nothing. Your tests are covered 100% at the pharmacy counter. |
Easiest way to request reimbursement is to submit the receipts online at OptumRx.com. Click here to submit online. You will need your receipt, the NDC number from the test kit box, and information from your OptumRx id card. You may request reimbursement for multiple test kits at once. However, each covered member will need to submit a separate request. A mail-in claim form is also available if you prefer not to submit online.

OPTION 3: Test kits are also available at $0 cost through the OptumStore. Kits purchased through the OptumStore will be mailed to your home. A link to the OptumStore is provided when you log into your optumrx.com account. Certain restrictions apply.

If you need a replacement OptumRx id card, contact OptumRx at 855-312-2307. If you are a registered user at optumrx.com, replacement cards as well as a temporary id card can be requested online.
