The deadline to complete the Reduced Deductible incentive (Online Health Assessment & Annual Physical) was August 31, 2024. You can see if you have been placed in the standard or incented deductible plan for 2025 by logging into fccBenefits.com, selecting My Plans, and clicking “As of 01/01/2025” from the dropdown menu. The first box will indicate which plan (standard or incented) you are in.
 
The deadline to submit an appeal for the 2025 deductible plan was December 22, 2024.
